Accelerated testing can only forecast so much, because operating conditions are so diverse that components will fail in the field regardless how you try to model its use in the lab.

The biggest hiccups usually take place when you change the chassis and powertrains....instead of holding the design up for further R and D work, it makes more economic sense to
the manufacturer to release the design to manufacturing and running a recall program later than to fix the underlying issue.

Notice how GM quietly moved the trucks fuel tanks within the framerails? How Ford quietly revamped the Explorer to have a wider track?

If production lines have to stop, they lose money.
